GCBHS Outreach Case Worker - SUD in Amelia, Ohio

GCBHS has an immediate opening for a Path Case Worker with expertise in SUD to help adults who are experiencing severe mental illness, substance use disorder, and homelessness in Amelia, OH. Position Responsibilities: Strives to attain the goals as written in their job performance evaluations and abide by the standards as set forth in GCB's Compliance Plan. Makes face-to-face contact with individuals who are homeless throughout Hamilton County to identify potential clients for the program (includes making contact with homeless persons on the streets, parks, soup kitchens, shelters, health care facilities, jails, places not meant for human habitation, etc.). Assesses individuals who are homeless for substance use disorders. Makes multiple contacts as needed and provides basic needs items to engage homeless individuals. Interacts with agencies, service providers, and other community contacts to solicit and receive referrals. Develops psychosocial profile through interviews with individuals and information collected from previous service providers. Identifies and connects individuals served to services aimed at addressing substance use disorder as well as other basic needs, including medical services, benefits, and housing. Attends staff meetings and training sessions as assigned. Position Requirements: High School Diploma or Equivalent required Related experience working with persons experiencing homelessness, mental illness, and/or substance use disorder The position requires frequent driving of the employee's personal vehicle.
