Shift: Days
Pay: $31.61 - $47.41 / hour. Pay is dependent on applicant's relevant experience
Summary:
This position is for our "Housing Transitions Team". This Social Worker works with patients of UC Health who are unhoused who may cross the Emergency Department, Inpatient or Outpatient clinics.
Key responsibilities include, but are not limited to:
Conducting assessments and developing intervention plans to help patients manage social, emotional, economic, and environmental challenges.
Facilitating transitions from acute care (hospitalization) to initial community placements, such as emergency shelters, respite programs, safe outdoor spaces, protective action respite facilities, and other transitional housing options.
Providing short-term case management and advocacy to support patients in securing stable housing and community placements.
Assisting patients with follow-up appointments and ensuring connection to appropriate levels of care and treatment.
Collaborating within a multidisciplinary team of social workers, nurses, care coordinators, physicians, and other healthcare professionals.
Requirements:
- Minimum Required Education: Master's degree in Social Work.
Licensure/Certification: State licensure as a Licensed Clinical Social Worker (LCSW).
