Nassau Suffolk Medical, P.C. is hiring independent contractor Speech Therapists to provide therapeutic treatment to adults diagnosed with physical, developmental and intellectual disabilities residing in Adults and Children with Learning and Developmental Disabilities’ (ACLD) group homes located throughout Nassau and Suffolk counties. This is a fee-for-service position with reimbursement for direct patient contact only.

Requirements:

• Must have Medicare and Medicaid provider numbers and a National Provider Identifier (NPI)

• M.A./M.S. in the specified field

• Licensed by and currently registered to practice in New York State

• Malpractice insurance

• Prior experience working with adults diagnosed with developmental disabilities

• Excellent communication skills for both parent and coworker interaction along with strong written skills.

• Prior experience with electronic medical record system preferred (ex. MediSked)

Responsibilities:

• Develop plan of treatment with specific goals in collaboration with the patient, family, physician and health team members.

• Provides direct service in group home setting.

• Document all clinical evaluations, treatments and patient’s response to therapeutic interventions within 24-hours of treatment in electronic medical record system.

• Continually re-evaluate patient’s condition and modify the plan of treatment as appropriate. Submit quarterly treatment plan for approval.

Schedule:

Flexible based on caseload. Patients are available weekday afternoons and evenings and during day hours on weekends. You schedule appointments directly with patient and group home management.

Fee-for-Service compensation based on sessions completed.

$36 per ½ hour speech therapy session $24 per 20 minute physical / occupational therapy session
