Behavioral Health Nurse Practitioner Consultant - PMHNP

The PMHNP Consultant supports the collaborative integrative care team: provider, patient, social worker, and nurses through regular consultation on an assigned caseload of patients. The behavioral health nurse practitioner consultant advises the primary care provider on appropriate medications and other treatment strategies, also providing the care team with educational support on diagnosing and managing behavioral health as well as substance use conditions. The PMHNP consultant attends weekly behavioral health team meetings, provides coverage for team absences, and provides behavioral health education to team as needed.

Responsibilities:

  • Provide regularly scheduled (usually weekly) caseload consultation to interdisciplinary care team. Case review consultations focus primarily on patients who are new to the caseload or who are not improving as expected under their current treatment plan. Case reviews may be conducted by telephone, or video.

  • Manage a panel of patients providing a full range of mental health services including behavioral health screenings, intake assessments, and follow up visits as needed.

  • Work with the interdisciplinary care team to oversee the status and population health of their assigned patient caseload. The behavioral health consultant uses a dashboard within the Electronic Medical Record (EMR) to track all assigned patients and prioritize patients for case review, follow up and treatment adjustment.

  • Recommend any treatment adjustments through the case review process or in ad-hoc consultations, including medication recommendations and other interventions, within 3 business days of consultation with interdisciplinary team (IDT) members.

  • Advise on treatment for patients who may need more intensive or more specialized mental health care, supporting treatment in the chronic care setting until patients can be engaged in specialized care as appropriate.

  • Clearly communicate the limitations of the case review consultation and treatment recommendations if the patient is not evaluated in person.

  • Direct evaluation of patients with significant diagnostic or therapeutic challenges who are identified in discussion with the patient’s provider. Such consultation may be provided in person or via telemedicine (video or telephone).

  • Development and delivery of in-service training for providers and other IDT staff regarding recognition and treatment of behavioral health conditions and substance use disorders in primary care.

Qualifications:

  • Nurse practitioner with advanced education/training and board certified as a PMHNP

  • Minimum of 5 or more years of NP experience, working independently in Behavioral Health and Geriatrics

  • Active and unrestricted NP license required in respective states TX, PA, AL, TN with ability to be credentialed in other states

  • Experience in both medication management as well as therapy

  • DEA licensure and prescriptive authority

  • Electronic Health Record experience-Epic preferred

  • Experience in coaching, training and educating staff

  • Experience working in an integrated behavioral health model

  • Excellent verbal and written communication skills

For this position, we anticipate offering an annual salary of 101,700 - 169,500 USD / yearly, depending on relevant factors, including experience and geographic location.

This role is also anticipated to be eligible to participate in an annual bonus plan.
