Bozeman Health Case Management Social Worker III (Casual Call) in Bozeman, Montana

Position Summary:

The Case Management Social Worker III intervenes with patients who have complex psychosocial needs and require assistance with eligibility determination for social programs and funding sources and qualify for community assistance from a variety of special funds and agencies. Offers crisis intervention to patients and families with psychosocial needs and coordinates and facilitates the development of a discharge plan of care for high-risk patient populations. Receives referrals for individuals from at-risk populations from interdisciplinary team members (including physicians, Case Managers, staff nurses and other members of the care team). Collaborates with other professionals to evaluate the patient’s medical or physical condition in order to assess patient needs. Supports family members to assist them in understanding, dealing with, and supporting the patient. Assures a cost- and quality-effective discharge, and promotes patients’ rights and maximum benefits from health care services. Remains actively involved in professional growth and promotion of social work roles and values. Initiates and maintains positive relationships with patients, customers, and co-workers.

Minimum Qualifications:

Required

  • Master’s Degree in Social Work

  • Montana Licensure (Licensed Clinical Social Worker or Licensed Master’s Degree in Social Work)

  • Five years’ experience providing social services in an acute hospital setting

  • Essential Job Functions:

In addition to the essential functions of the job listed below, employees must have on-time completion of all required education as assigned per DNV requirements, Bozeman Health policy, and other registry requirements.

  • Assesses patient’s and family’s psychosocial risk factors through evaluation of prior functioning levels, appropriateness and adequacy of support systems, reaction to illness and ability to cope.

  • Intervenes with patients and families regarding emotional, social, and financial consequences of illness and/or disability; accesses and mobilizes family/community resources to meet identified needs.

  • Provides intervention in cases involving child abuse/neglect, domestic violence, elderly abuse, institutional abuse and sexual assault.

  • Advocates for patient and family empowerment and independence to make autonomous health care decisions and access needed services within the health care system.

  • Participates in discharge planning activities for complex patients, in order to ensure a timely discharge and to provide appropriate linkage with post-discharge care providers.

  • Facilitates communication and coordinates all care of the high risk patient from admission through discharge. Deals with families exhibiting complex family dynamics that impact directly on patient care and discharge.

  • Confers with health team members to identify patients in need of intensive post-acute services and confirms probable post-acute needs through supplemental assessment, collaborating with patient, family, physician, and team members to develop a transition plan appropriate to the patient's needs and financial capabilities. Determines patient's eligibility for post-acute services within 24 hours of inpatient admission and ensures that discharge plan implementation activities are conducted to effect a safe and timely transition to the community.

  • Initiates referrals to the appropriate Post-Acute Care Providers to expedite care, treatment, and services; utilizes evidence-based clinical protocols when warranted by patient's diagnosis, and seeks input from clinical experts to explore acceptable alternatives to treatment plan.

  • Convenes and conducts interdisciplinary conferences to solicit input concerning the patient's treatment/discharge plan from clinical team members collaborating with physician, patient, family members and nurses to identify probable post-acute needs and ensuring consistent data capture to identify trends/problems related to delivery-of-care delays and avoidable days.

  • Issue Medicare HINN/ABN or other regulatory notices to patients when deemed necessary.

  • Stroke Competency: Complies with hospital policies, procedures, protocols, and guidelines related to care for patients with hyper acute and acute stroke. Participates in one stroke awareness/recognition activity annually.

Physical Requirements

  • Handling Challenging Situations: The role may involve dealing with upset individuals, requiring the ability to de-escalate situations and work effectively with frustrated patients, families, or employees

  • Repetitive Tasks: Ability to perform repetitive tasks as needed to fulfill job responsibilities

  • Bloodborne pathogens, such as blood, bodily fluids, or tissues.

  • Radiation in settings where medical imaging procedures are performed

  • Various chemicals and medications used in healthcare settings. Job tasks may involve handling cleaning products, disinfectants, and other substances.

  • Infectious diseases due to contact with patients in areas that may have contagious illnesses
