Care Manager

Description

A Care Manager forms a relationship and follows up with patients to ensure that they are compliant with their medical care. In addition, the care manager will liaise with providers and family members as needed to ensure that the patient has access to quality medical care.

Duties and Responsibilities

· TeleHealth

o Follows a specified panel of patients with chronic medical conditions on a regular basis

o Monitors Remote Patient Monitoring (RPM) data and evaluates for changes that require intervention as directed by facility policy

o Serves as a contact point for their patients to access their primary care provider

o Reports lab results to patients as directed by the provider

o Encourages and assists patients in making appointments with primary care and specialty care providers at specified intervals

o Provides encouragement and aids patients in reaching their health goals

· Clinical responsibilities

o Assists with patient intake and data collection, including medical history and vitals, during in-person visits to the facility

o Assists with minor procedures, including blood draws and sample collections, within scope of practice

o Meeting with patients to form a positive relationship conducive to quality medical care.

· Other responsibilities as requested by the supervisor and within scope of practice

Qualifications

· 3 years experience in health care

· High School Diploma required, College Education preferred

· Experience with Electronic Medical Record (EMR) system preferred
