JOB SUMMARY

The Licensed Practical Nurse, as a member of the Resource Team, collaborates with Clinical Specialists, Care Coordinators, and Quality Assurance Auditor’s to identify current or potential health disparities, in turn providing health promotion activities, referrals, assessments, and valuable expertise around the combination of social determinants of health, behavioral, and physical health diagnoses to improve the lives of the individuals we serve.

ESSENTIAL FUNCTIONS

  • Works in collaboration with other Clinical Resource Team members on case reviews to ensure continuity of care.

  • Facilitation of case review meetings with Care Coordinators and applicable team members.

  • Serve as a clinical resource for non-medical team members, providing education and clarification on medical terminology, procedures, and diagnosis.

  • Ensure appropriate provider specialist linkages are in place based on IDD, health, and mental health diagnosis.

  • Act as a role model and mentor to care coordination teams.

  • Support the continuous improvement of healthcare services, contributing to the enhancement of person safety and the optimization of positive health outcomes.

  • Provide health literacy resources and training for team members.

  • Assist Care Coordinators with obtaining and interpreting medical records.

  • Determining the necessity and streamlining access to preventive health screenings, tailored according to age and gender.

  • Administer acuity scale to drive action plan for case review follow up.

KNOWLEDGE, SKILLS & ABILITIES

  • Knowledge of developmental disabilities, chronic disease, and social determinants of health

  • Operate in a trauma-informed manner.

  • Ability to engage people with Intellectual/Developmental Disabilities

  • Ability to speak and convey medical information to groups of people.

  • Knowledge of person-centered planning regulations

  • Ability to build relationships and effectively communicate

  • Excellent written and verbal communication skills

  • Demonstrates cultural competence

  • Demonstrates ethical and professional responsibilities and boundaries

  • Demonstrates capacity to use Health Information Technology to facilitate communication

  • Strong computer literacy

  • Knowledge of confidentiality regulations/HIPPA

  • Ability to prioritize and complete work in timely manner

  • Ability to work independently

  • Ability to provide guidance in a clear, concise manner

  • Knowledge of trauma-informed practice

  • Proactively approaches professional responsibilities

  • Familiarity with person-first, person-centered planning concepts

  • Ability to professionally interact with various levels of teams and leadership.

  • Ability to offer constructive feedback and recommendations for improvement.

EDUCATION & EXPERIENCE

  • LPN with 2+ years serving at risk populations required.

  • At least 2 years of experience working with people with intellectual/developmental disabilities p referred.
