Nurse Practitioner (NP) - Weekday Schedule in Cleveland Heights, Ohio

The Certified Nurse Practitioner performs hospice and palliative care patient evaluations in private residences, nursing facilities, and assisted living facilities, provides inpatient and outpatient palliative care consults as well as a liaison with attending physicians and medical directors.

Position Qualifications:

  • 1-2 years of nursing experience in acute care, skilled home health, hospice or another related environment

  • New grad Nurse Practitioners considered

  • Home Health, Palliative Care, or Hospice Experience preferred

  • Masters of Science in Nursing.

  • Active unencumbered OH NP license.

  • Current driver's license and auto liability coverage.

  • Detailed assessment skills as part of this role require recertification for Hospice patients.

  • Interest in a community-based role that requires travel between patient visits.

  • Drug Enforcement Agency (DEA) number Required

  • National Provider Identifier (NPI) Required.

Shift Availability:

  • Monday to Friday 8:30 AM to 5:00 PM 

  • Required phone support on call 1 weekday per week and 1 weekend per month
