RN Assessment Coordinator

Cabell Health Care Center joined the AMFM Family July 1, 2014 and is a 90 person care center specializing in skilled nursing, short-term rehabilitation to home, and long-term care services.

Position Summary: - Coordinate, facilitate and ensure the accurate and timely completion of the RAI process (MDS, RAPs, Care Plans) for all patients according to CMS, OBRA regulations, Medicare, Medicaid and Commercial Insurances as well as company policies and procedures

Education and Experience Requirements:

  • Graduate of an accredited school for registered professional nurses and must possess a current, unencumbered, active licensed in the state of West Virginia.

  • Must possess and maintain current CPR certification, or acquire a CPR certification within 90 days of date of hire.

  • Prefer experience with both MDS and PPS.

  • Must possess excellent clinical assessment and organizational skills

Other Specific Requirements:

  • Must possess the ability to make independent decisions when circumstances warrant such action

  • Must be knowledgeable of reimbursement regulations and nursing practices and procedures, as well as laws, regulations, and guidelines pertaining to long-term care administration.

  • Must possess the ability to deal tactfully with staff, residents, visitors and the general public.

  • Must be knowledgeable of computer systems, system applications, and other office equipment.

  • Occasional travel will be required and occasional overnight travel may be required.

Work Schedule Routinely: work five days a week during the day shift and as the need arises on other occasions, including weekends and off hours. May be required to provide On Call coverage during non-working time by being available to provide assistance and direction to facility staff. This is a salaried position.

Physical Requirements: Must be able to lift in excess of 40 pounds. Must be able to cope with the mental and emotional stress of the position. Must meet the general health requirements set forth by the policies of this facility which include a physical examination upon hire.
