This Care Navigator position is responsible for coordination of care both Pre and Post HouseCalls Visits to ensure accurate and up to date information is present prior to the HouseCalls visit and to assist with Social Determinates of Health (SDoH) including communication with Providers (both internal and external) as well as members and their families during and after their HouseCalls visit. The HouseCalls Care Navigators will work in a call center environment virtually from their own home to provide immediate care coordination needs via telephonic incoming calls.

This position is full-time. Employees are required to have flexibility to work any of our 8-hour shift schedules during our normal business hours of 12:30 pm - 9:00pm PST. It may be necessary, given the business need, to work occasional overtime.

We offer 1 month of training. Training will be conducted virtually from your home.

Primary Responsibilities:

  • Function as an advocate both pre and post HouseCalls visit

  • Educates member on gaps in care and assists with scheduling provider appointments

  • Ensures documentation in EMR utilized by HouseCalls Practitioners is up to date and accurate prior to the visit to assist with a smooth and engaging yearly visit

  • Gathers and charts demographic data from members prior to the visit

  • Assesses functional and social needs of members and coordinate services, treatments, and/or referrals

  • Work collaboratively the multidisciplinary team to engage resources and strategies to address medical, functional, and social barriers to care

  • Identify, refer, and coordinate available community resources to address non-medical needs and ensure patient access to services

  • Utilizes approved platforms to research community resources

  • Completely and accurately document in patient’s electronic (UnitedHealth Care) medical record

  • Provide patients and family members with education regarding the need for follow up as appropriate

  • Actively participate in organizational quality initiatives

  • Demonstrates a clam, organized and courteous demeanor telephonically to members and Practitioners

  • Demonstrate a commitment to the mission, core values and goals of UnitedHealthcare and its healthcare delivery including the ability to integrate values of compassion, integrity, performance

Required Qualifications:

  • High School Diploma / GED (or higher)

  • 1+ years of call center experience (Inbound and Outbound)

  • 2+ years in healthcare-related field working with medical terminology and/or taking patients histories

  • Ability to work an 8 hour shift during our normal business hours of 12:30 pm to 9:00 pm PST

Preferred Qualifications:

  • Bachelor's degree (or higher) in social work, licensed practical nurse (LPN), CNA/HHA or HS graduate with 3 years of experience working within the community health setting in a health care role

  • 3+ years of experience in working with at risk populations

  • CHW Certification

  • Experience working in managed care

  • Bilingual fluency in Spanish and English

  • Certified Nursing Assistant / Home Health Aide, or Medical Assistant

Telecommuting Requirements:

  • Required to have a dedicated work area established that is separated from other living areas and provides information privacy

  • Ability to keep all company sensitive documents secure (if applicable)

  • Must live in a location that can receive a UnitedHealth Group approved high-speed internet connection or leverage an existing high-speed internet service

Soft Skills:

  • Ability to work with diverse care teams

  • Knowledge of culture and values of community

  • Exhibit independent judgment with the ability to consult and engage with other team members for improved patient outcomes.

  • Telephone etiquette

  • Excellent organizational skills

  • Must be able and comfortable with maintaining metrics and goals within the department

  • Must be comfortable working on the phone and multiple systems on the computer simultaneously while assisting members

Colorado, Connecticut, Nevada or New York City Residents Only: The salary range for Colorado residents is $18.17 - $32.26. The salary range for Connecticut / Nevada / New York City residents is $20.00 - $35.53.
