Job Summary and Qualifications

This position supports the mission of Presbyterian/St Luke’s Medical Center of optimizing quality of life of all those served by expanding beyond the traditional focus on medical needs to the needs of the whole person. The Long Term Follow Up Coordinator is responsible for providing life-long support for patients who have had a hematopoietic cell transplant. The Long Term Follow Up Coordinator ensures that every transplant patient receives the appropriate education and information prior to being discharged from the transplant program. The Long Term Follow Up Coordinator will remain in contact with patients to monitor the status of his or her disease for which a transplant was done, screen for and document chronic graft-versus-host disease, and ensure appropriate follow-up is provided to patients based on the type of transplant. The Long Term Follow Up Coordinator completes appropriate data collection forms, performs necessary reporting to NMBT, CIBMTR, FACT, and other regulatory agencies. The Long Term Follow Up Coordinator works in conjunction with other members of the transplant team to prepare for FACT inspection. The Long Term Follow Up Coordinator will track and complete gap analysis and audit LTFU processes and policies to ensure compliance and evidenced based care is being provided.

II. Position Requirements:

A. Licensure/Certification/Registration: Current licensure in the State of Colorado as a Registered Nurse, or current active multistate RN license; BLS – Healthcare provider. OCN or BMT-CN certification within 18 months of hire.

B. Education: Graduate of an accredited school of nursing.

C. Experience: Three to five years experience in hematology-oncology or bone marrow transplant care preferred.

D. Special Qualifications: Ability to communicate effectively with patients, families, physicians, and other health care team members. Ability to manage a team and work with others within a team to ensure quality patient care. Strong critical thinking skills. Desire to provide compassionate, holistic care.

III. Degree of supervision required: Involves general guidance and direction by the Cancer Care Director and the Lead Allo Coordinator. Employee will be expected to perform most job duties independently and in accordance with established departmental and hospital policies and procedures.

IV. Ages of Patients Served: This position requires knowledge of growth and developmental needs of patients and families. Personnel in this position must recognize and respond to the behaviors of patients and families in each age category. The employee must be able to demonstrate the special skills required to care for patients and their families according to standards. (The skills and knowledge needed to provide such care may be gained through education, training or experience.)
