Satellite Healthcare Nephrology Dietitian in Fremont, California

Job Summary:
The Nephrology Dietitian is responsible for supporting the nutritional health of patients at Satellite Healthcare. The Nephrology Dietitian assesses the nutritional needs of each patient, engages patients and their families or caregivers in establishing goals and developing individualized nutrition plans, and monitors understanding and response to therapeutic nutrition plans. The Nephrology Dietitian will fulfill the Satellite Dialysis Mission Statement: "Making Life Better for Those Living with Kidney Disease"
The Nephrology Dietitian may function in various roles dependent on experience, responsibility, and scope aligned with the Academy of Nutrition and Dietetics Standards of Practice and Professional Performance:
· Nephrology Dietitian I (Competent) - Registered Dietitian with a minimum of one (1) year of clinical experience who is newer to the field of nephrology and dialysis. At the Nephrology Dietitian I level, an RD is learning the foundations of nephrology nutrition and is developing skills for safe and effective nephrology practice.
Essential Functions:
· Evaluates patient’s nutrition needs/status to develop an individualized nutrition plan. Instructs patient/family/care provider as appropriate on recommended nutrition plan using an individualized approach
· Establishes nutritional priorities and develops a team action plan in conjunction with RN, MD, MSW, and patient
· Provides ongoing nutrition assessment of patients; modifies nutrition goals/dietary recommendations as needed on a regular basis per Satellite Healthcare Standards of Practice
· Communicates nutrition information/status to other treatment providers to ensure ongoing nutrition care when patients transfer to another facility/modality or require concurrent care/evaluation from outside agencies, hospitals, clinics, etc.
· Participates actively in Interdisciplinary Team activities, including patient care conferences, QAPI meetings/projects, physician clinics, and home visits
· Carries out MD renal dietitian protocol orders
· Maintains nutrition-related documentation in SPIN
· Provides nutrition-related in-service education for center staff as appropriate/requested
· Responsible for driving the Satellite Healthcare culture through values and customer service standards
· Accountable for outstanding customer service to all external and internal customers
· Develops and maintains effective relationships through effective and timely communication
· Takes initiative and action to respond, resolve and follow up regarding customer service issues with all customers in a timely manner
Communications
· Communicates significant patient information to the appropriate team members in a clear, concise, and accurate manner
· Collaborates with the Interdisciplinary Health Care Team on shared responsibilities
· Contributes and promotes teamwork to create a harmonious and constructive working relationship with all staff by offering information, advice, and assistance in a positive, courteous, and cooperative manner
· Participates in self and/or peer evaluations
· Ensures the confidentiality of patient and employee information
· Understands and uses Satellite’s communication techniques:

Minimum Education & Experience:
· Registered Dietitian (includes Baccalaureate or Advanced degree)
· One (1) year experience as a Registered Dietitian in clinical nutrition; 2 years preferred
Any combination of education and experience that would likely provide the required knowledge, skills, and abilities as well as possession of any required licenses or certifications is qualifying
Minimum Knowledge, Skills & Abilities:
· Able to read, write, speak, understand and satisfactorily communicate with others in English in person, over the phone and via email
Based on California’s new pay transparency law the hour wage will need to be listed on posted positions.
The compensation for the role will depend on several factors including the candidate’s qualifications, work experience, competencies, and skills and may fall outside of the range shown.
Salary Range - $81,586 - $97,904
