Duties:\ - Conduct outreach activities to identify individuals in need of services\ - Provide information and resources to individuals regarding available programs and services\ - Assist individuals in accessing appropriate resources and support systems\ - Conduct assessments to determine the needs and eligibility of individuals\ - Collaborate with community organizations and agencies to coordinate services\ - Maintain accurate and up-to-date records of client interactions and services provided\ - Participate in team meetings and trainings as required

Skills:\ - Strong communication skills, both verbal and written\ - Familiarity with medical terminology and Medicare regulations\ - Ability to collect and analyze data for reporting purposes\ - Proficiency in motivational interviewing techniques
