+-----------------------------------------------------------------------+ | Position Summary: | | | | Responsible for individualized client services provided in a | | community/residential setting.  Duties include assessing, planning, | | implementing, monitoring, and evaluating client behaviors and | | outcomes while ensuring appropriate level/type of services are | | provided. | +-----------------------------------------------------------------------+ | Position Description: | | | | - With close* *supervision; conducts interviews and administers | | assessments and screening instruments | | - Maintain caseload and productivity by ensuring regular contact | | with clients to monitor and assess progress | | - Write service plans, review progress and determine updates needed | | in collaboration with client | | - Utilize core correctional practices while assisting clients in | | effective community re-integration | | - Deliver on-going services to clients which may include: crisis | | intervention, advocacy, medication monitoring, transportation | | assistance, financial budgeting, housing assistance, and referral | | and linkage to community resources as needed | | - Ensure compliance with applicable standards and agency policies | | - Document all clinical activities to meet specific program, funder | | and regulatory criteria timely and with accuracy | | - Participate in team meetings and assist in making recommendations | | for client status changes | | - Lead educational groups and/or facilitate group curriculum | | - May require transportation of clients | | - Adhere to professional standards as outlined by protocols, rules | | and regulations | | - Other duties as assigned | +-----------------------------------------------------------------------+

Qualifications
