Description:

• Assist patients, medical practices with any records requests, following HIPAA protocols for all information disclosed. • Scan, file, and import into the patients’ correct file in company EMR system the medical record copying requests, medical record charts, and outside medical records received via fax, mail or CD. • Must read all medical record copying requests and Subpoenas completely. Convert medical records requested into a PDF format from company EMR systems, copy paper chart, and/or order medical record chart from outside storage vendor. • Process medical records copying request received by interoffice mail, fax, US Mail and personal serve from copy service. Date stamp all incoming medical records copying requests. Comply with medical records requirements, including constraints and confidentiality issues. • Daily logs in all medical records into department tracking system. Unsure what this means Enter accurate and pertinent information such as patient’s information, requestor address, telephone, and fax number. Must be consistent when checking patient signatures or Proof of Service of Subpoena, all adults 18 years and older sign for themselves or must provide legal documentation to support authority. Authorized parent can sign for child(ren). Medical records copying request or Subpoena must be filled out completely. • Correctly identify patient on medical record copying request by searching for correct medical record number in company EMR system. • Notify patients of copying procedures and charges associated with copying medical records. Messages to be left for patients only. Speak with patient or patient’s legal representative as mandated by law. • Identify improvement opportunities, implement countermeasures and escalate to appropriate management resource as needed. • Checking work logs for missing operative reports and obtaining those reports from the correct platforms • Matching any documents to patients charts for provider review (Consultations, Labs, etc.). • Scans and files lab reports, correspondence, physician dictation/notes, progress notes, reports, and other approved documents in charts within designated time frames. • Faxing Office Visit Reports • Ensures fulfillment of all mailed-in and faxed requests for medical records from insurance companies, managed care plans, hospitals, attorneys, patients and other physicians when appropriate releases are provided. • Verifies authenticity of subpoenas and release of information forms and processes requests according to state guidelines. • Working on the EMR System E Clinical works

Additional Skills & Qualifications:

1+ years recent Medical Office experience 1+ years of medical records experience Medical Assistant Certificate REQUIRED BLS required and/or CPR certified -Medical Terminology -Medication knowledge OR Pharmacy knowledge required -EMR experience -Strong computer skills -Strong phone etiquette -Strong communication with patients, providers, pharmacies, labs -Detail oriented -Ability to fill prescription requests
