Dartmouth Health Nurse Navigator (RN), Pediatric Pulmonology clinic in Lebanon, New Hampshire

Overview

32 hours/week

In collaboration with a multidisciplinary team, the Nurse Navigator – RN serves as an advocate for patients with serious illness by providing care coordination, education and support to patients, families and caregivers throughout the care continuum to ensure high quality of care across all care delivery sites. Responsible and accountable for the management of resources and serves as a liaison between the patient and family and all physicians, internal and external healthcare providers, support network members and the wider healthcare community.

Responsibilities

  • Serves as a patient advocate for patients with serious illness. Initiates contact with the patient to introduce the navigation program, the navigator role and be accessible to patients and families throughout the care continuum. Responds to patient challenges/barriers to care until resolution is achieved.

  • Assesses patients’ medical, social, psychosocial, and other care needs matching unmet needs and barriers to care with appropriate services, referrals and support services including health professionals, providers and financial services.

  • Collaborates with the care team to assist patients in understanding their diagnosis, treatment options and the resources available, including education, clinical research studies, and technologic advances.

  • Develops or utilizes appropriate assessment tools to promote a consistent, holistic plan of care.

  • Attends Interdisciplinary Team meetings and/or other meetings as necessary to ensures that appropriate patient data is available, complete and accurate. Serves as the patient advocate and interdisciplinary team member at these meetings.

  • Acts as a liaison between patients, families and caregivers and the providers to optimize patient outcomes.

  • Drives process improvement through collaboration with others to determine programmatic and operational opportunities for improvement, makes appropriate recommendations for changes, and assists in delivering program improvement.

  • Establishes and maintains positive working relationships with key internal and external customers.

  • Performs other duties as required or assigned.

Qualifications

  • Graduate from an accredited Nursing Program required.

  • Bachelor of Science Degree in Nursing (BSN) required; Master's preferred.

  • Three years of applicable specialty experience in an acute or ambulatory care setting preferred.

  • Strong organizational skills and the ability to prioritize and reprioritize

  • Ability to develop collaborative relationships internally and externally

  • Basic computer skills

  • Ability to work autonomously

Required Licensure/Certifications

  • Licensed Registered Nurse with New Hampshire eligibility.

  • BLS for Healthcare Provider

  • Nurse Navigation Certification required within two years of hire.
