Coleman Health Services is a nationally recognized not-for-profit provider of behavioral 

health and rehabilitation programs to foster recovery, build independence, and change 

destinies for individuals, families, and businesses in our community. We are seeking a 

team player for our Case Management Program in Allen and Hardin Counties, Ohio. 

Responsibilities include  

• Establishing client outreach plans,  
• Providing support in development and accomplishment of goals 
• Establishing community/client relationships.  
• Assist clients with daily living skills 
• Medication monitoring 

• Following their Individual Service Plans.  

   

Candidates must have. 

• Associates degree in human services or related field:  
• Willing to work a flexible schedule to meet the needs of our customers.  
• Have experience working with severely mentally disabled individuals 
• have and maintain excellent driving record.  
• Individual must have the following: a NPI number, and eligibility to apply for an Ohio Medicaid number.  

• BA/BS in Social Services preferred.
