The Assertive Community Treatment (ACT) case manager works with individuals with a severe mental illness and often, a co-occurring substance use disorder. The case manager works collaboratively with other ACT team members to provide services and linkage to community resources and assistance with meeting basic needs for these individuals.

Responsibilities:

• Provide case management services to individuals and families in need
• Conduct assessments to determine client needs and develop appropriate service plans
• Collaborate with clients to set goals and monitor progress towards achieving them
• Connect clients with community resources and support services
• Advocate for clients and assist them in navigating complex systems
• Provide crisis intervention and support during emergencies or difficult situations
• Maintain accurate and up-to-date case records and documentation

-Facilitate weekly co-occurring disorders group session

Qualifications:

• Experience working in behavioral health or social services field preferred
• Knowledge of motivational interviewing techniques and crisis management strategies
• Strong communication, interpersonal, and problem-solving skills
• Ability to work effectively with diverse populations and demonstrate cultural sensitivity
• Excellent organizational skills and attention to detail
• Ability to work independently as well as part of a team
