UPCAP  is accepting applications from Licensed Social Workers *and *Registered Nurses interested in providing Person-centered Care Management/Supports Coordination for a nursing-home eligible population wishing to remain in their own homes.  This non-clinical position is based in Marquette, MI and provides services throughout Alger and Marquette Counties.

UPCAP offers travel reimbursement, a competitive salary and full fringe benefit package including health, dental and vision insurance, long term disability and retirement.  NO weekends, on call or night shifts PLUS ** Option to work from home.

Must pass a criminal background check and have a valid drivers license with proof of insurance.
