Requirements for PT : Graduate of accredited program in Physical Therapy with minimum of a Bachelor's degree. Holds a current Physical Therapy license from the State of Ohio in good standing. Maintains CPR Certification.

Requirements for OT: Graduate of accredited program in Occupational Therapy with minimum of a Bachelor's degree. Holds a current Occupational Therapy license from the State of Ohio in good standing. Maintains CPR Certification.

Requirements for SLP: Graduate of accredited program in Speech Language Pathology program. Holds a State of Ohio license in good standing with a Certificate of Clinical Competence (CCC) from the American Speech, Language, and Hearing Association or a conditional license under a supervised Professional Experience Plan approved by the Ohio Board. Current CPR certification.
