Case Manager Position Summary:

The Case Manager works under the guidance of the Clinical Program Director and Assistant Clinical Program Director, while mentoring case management staff. The Case Manager collaborates with other healthcare team members to coordinate and facilitate the effective delivery of quality services to Compass patients.

The primary role of the Case Manager is to coordinate the aftercare needs of each client based on a person-centered process which is driven by an individualized treatment plan that functions to maintain the individual’s well-being and prioritize treatment goals and objectives. The Case Manager functions as an expert in the community and military resources and participates as an independent member of the multi-disciplinary treatment team. The Case Manager uses that knowledge to assist clients in accessing the network of resources that will help maintain their community and military status and promote further growth to maximize functional independence. This level of case management while clients are in residential levels of care will begin to transition towards discharge as more stability is achieved in the individual’s life and as the individual moves toward successful completion of substance use and mental health treatment.

Qualifications

Case Manager Job Requirements:

  • Bachelors degree required, Master’s degree preferred.

  • Minimum of 2 years of experience in case management related work.

  • Completion of Crisis Prevention and Intervention (CPI) training within 30 days of employment
