FCA is excited to announce an opportunity for an undergraduate Intern student for our Linkage Program!

The Link-Age Program is a grant funded program by NYS Office for Mental Health in a triple partnership with Family & Children's Association, Central Nassau Guidance & Counseling Center, and NYS Office for Aging. The program is focused on providing short-term intensive case management, counseling, health and psychoeducation, and peer services. We provide linkage to long term care, community supports and services to seniors 55 years+, residing in Nassau County that are facing behavioral health concerns, substance use disorders, challenges related to aging, disconnection from services, and struggling to access/connect to medical care.

As a Link-Age Intern, you will learn how to complete initial intake forms and assessments, gain an understanding of program operations in relation to grant funded requirements, learn about the value of data collection and assist with entering information into collection systems. There are opportunities to provide community education through outreach efforts and presentations. You will learn about the available resources and service providers in Nassau County for the aging population, have opportunities to network with other organizations and participate in professional development trainings.

Responsibilities

  • Provide a blend of telehealth and in-home visitations to clients while collaborating with program staff to address client's needs.

  • Contact referral made to the program to triage and assess eligibility, collect intake information

  • Perform assessments and screenings to identify clients struggling with mental health, substance use, loneliness, and aging needs.

  • Assist program staff with entering data and documents in the electronic health record system

  • Conduct follow up calls and surveys with clients that are 3 months post-discharge from the program

  • Opportunity to maintain a client case load and provide case management services to clients such as: assisting clients with completing forms/applications to apply for services & programs; provide information/referrals to clients as requested; address barriers to connecting with services/ resources.

  • Adhere case records, assessments, and statistics as required by the standards set by Family & Children's Association and NYS Office for Mental Health grant guidelines.

  • Participate in supervision and weekly Team Meetings; participate in agency trainings and workshops.

Qualifications

  • Mus be enrolled in a Bachelors level program for Social Work or related profession (2nd year)

We are unable to accept Practicum Internships

  • Valid New York State driver's license

  • Interest working with Geriatric/Older Adult or SMI population preferred.

Additional Information

  • Schedule: In office a minimum of twice a week

  • Office is open Mon-Fri 9am-5pm

  • Location-Garden City
