Our Ending Homelessness division is dedicated to providing safe, responsive housing solutions through a continuum of care that respects the dignity of each individual. We are currently seeking a compassionate and driven Case Manager to join our team in one of our site-based housing programs. About Avivo: Avivo provides a broad spectrum of services to help individuals and families achieve recovery, economic stability, and independence. In the Ending Homelessness division, we focus on pathways to stable housing using culturally responsive, trauma-informed practices centered on harm reduction and housing first principles. Role Overview: As a Case Manager in our site-based housing program, you will support a diverse range of single adults who have previously experienced long-term or chronic homelessness. Your role is pivotal in supporting residents with active substance use disorders and ensuring they have the stability and resources needed to sustain housing and engage in recovery. What You Will Do: Build impactful, professional relationships with clients, helping them set and reach personal goals through creative and client-centered planning. Conduct assessments, make appropriate referrals, and provide direct housing-related services. Foster strong relationships with landlords and other community partners to facilitate housing solutions. Complete necessary documentation and maintain accurate case files in compliance with our data quality standards. Actively participate in team meetings, provide support to colleagues, and contribute to a collaborative work environment. Qualifications Who You Are: You have a High School Diploma or GED; additional experience in social services, particularly with homeless populations, is highly valued. You possess a deep understanding of Harm Reduction and Housing First practices. Your communication skills are strong, and you can advocate effectively for diverse client needs. You are flexible, adaptable, and able to work in dynamic and challenging environments. You are driven by a passion for social justice and a belief in the intrinsic right to housing. Physical and Travel Requirements: Comfort with standard office duties and occasional physical activities such as moving light materials. Availability for flexible hours, including some evenings and weekends. Valid driver's license and ability to travel to various community sites as needed.
