The Developmental Disabilities Program procures and supports community-based services for people with developmental disabilities and/or their families. The DDP Targeted Case Manager is responsible for a comprehensive assessment and periodic reassessment of individual needs, the development of and periodic revision of the plan of care, referral and related activities to help the member obtain needed services, and monitoring and follow-up activities. Additional Information This position may be eligible for partial telework within the state of Montana. Specific conditions will be provided by the supervisor and must adhere to state policy. This position may be eligible for a statutory $1.50 an hour base pay increase effective July 1, 2024. This position is part time at 20 hours per week.
