State of Montana DDP Targeted Case Manager (14412) in Missoula, Montana

The Montana Department of Public Health and Human Services (DPHHS) has a career opportunity within the Behavioral Health & Developmental Disabilities Division. The Developmental Disabilities Program procures and supports community-based services for people with developmental disabilities and/or their families. The DDP Targeted Case Manager is responsible for a comprehensive assessment and periodic reassessment of individual needs, the development of and periodic revision of the plan of care, referral and related activities to help the member obtain needed services, and monitoring and follow-up activities. Qualifications for this Career Opportunity * * * Above average ability to follow written and verbal directions * Ability to communicate both orally and in writing. * Above average ability to recognize, analyze, and solve complex problems/dilemmas. * Ability to facilitate, negotiate and mediate problems as well as promote team consensus. * Above average initiative and flexibility skills and strong work ethic to be reliable and productive. * Ability to operate job related computer software and a variety of office equipment. * Meet minimum qualifications: o Bachelor’s degree in social work or a related field from an accredited college. o Minimum 2 years’ experience in human services, with at least one year’s experience in the field of developmental disabilities (DD). Additional Information This position may be eligible for partial telework within the state of Montana. Specific conditions will be provided by the supervisor and must adhere to state policy. This position may be eligible for a statutory $1.50 an hour base pay increase effective July 1, 2024. This position is part time at 20 hours per week.
