Covenant Health PERFORMANCE IMPROVEMENT/INJURY PREVENTION COORDINATOR in Nashua, New Hampshire

Summary

The trauma performance improvement/Injury prevention coordinator facilitates the trauma program performance improvement requirements through identification of concerns or departures from best practices. Collaboration with peers to affect change and monitoring ongoing initiatives. Local injury prevention programs based on registry data are also within the PIC position.

Essential Duties and Responsibilities

  • Supports and promotes the mission and values of Covenant Health Ministry.

  • Participate in state, regional, or national trauma performance improvement activities.

  • Attend NH state Trauma Medical Review Committee meetings.

  • Conduct community education programs based on demographic conditions.

  • Develop and maintain a positive public presence for the injury prevention program.

  • Participate in the Trauma Quality Improvement Program (TQIP).

  • Oversee the EMS outcome letter process and assess pre-hospital activities.

  • Ensure that St Joseph Hospital maintains nationally verified American College of Surgeons Level III Trauma Standards regarding performance improvement and other related standards.

  • Review and preparation of select cases for presentation at Trauma committee and TOPIC meetings, M and M.

  • Work to ensure all PIPS documents interface with the trauma registry.

  • Take initiative in planning, developing, organizing, and evaluation of the St Joseph Hospital Trauma Performance Improvement and Patient Safety (PIPS) Program.

  • Participate in the monthly interprofessional Trauma PIPS Committee. Will share appropriate data and document all information for the peer review protected minutes.

  • Develop and maintain monthly data reporting and tracking.

  • Oversee data validation efforts to ensure data integrity as it relates to PIPS activities.

  • Participate in annual review of applicable trauma registry inclusion criteria.

  • Data management and validation, report production to meet Trauma Center and regional quality management, trauma research, epidemiologic, and injury prevention initiatives.

  • Other duties as consistent with this role.

    Job Requirements

    Job Knowledge and Skills

  • A clinical background and knowledge set necessary to interpret/analyze patient care and outcomes.

  • Superior attention to detail with excellent verbal and written communication skills.

  • Strong ability to track and analyze data and make recommendation based on this data synthesis.

  • Ability to work independently and handle multiple ongoing tasks.

  • Ability to work well under tight timelines.

  • Ability to work as a part of a team serving multiple customers.

  • Ability to maintain confidentiality of PHI.

  • Positive attitude and customer focus

    Education and Experience

  • Active NH Registered Nurse or Paramedic license

  • Three (3) years of trauma care experience (Critical care, ED, floor, trauma program) preferred.

  • Three (3) years of recent clinical, data analysis, or quality improvement experience preferred.

  • Additional professional certifications, TNCC, TCRN, CSTR, CAISS preferred.

  • BSN preferred.

    An equivalent combination of education and experience which provides proficiency in the areas of responsibility listed above may be substituted for the above education and experience requirements.

    Working Conditions/Physical Demands

    Must possess the physical and mental abilities to perform the tasks normally associated with this position that involves a combination of sitting, standing, walking, bending, stooping, and reaching. Occasional lifting/carrying up to 25 lbs. Some stress related to high level of responsibility for quality care.
