Salary: $37,598 to $45,819 AnnuallySalary is based on education and experience

Job Description: Provides skills training to children and youth and to their parents/LAR(s) for children with behavioral/emotional disorders as identified and prescribed in the individuals care (recovery) plan. Also provides targeted case management including crisis prevention and management, monitoring, assessment and service planning and coordination for individuals on the caseload. Case Management includes assisting the child/youth with accessing medical, social, educational, and other appropriate services that will help the individual achieve a quality of life and family/community participation acceptable to the individual. This position negotiates and facilitates the array of services and supports needed to address the individuals personal goals and desired outcomes as identified through the TRR assessment and Recovery Plan; also sees that planned services and supports are implemented and appropriately documented. As Recovery Facilitator, coordinates treatment and advocates with other agencies, including schools, to assist the individual in reaching desired outcomes. Position also provides crisis prevention and management, monitoring, assessment, rehabilitative skills training, service planning and coordination for adults with mental illness in the Madison/Leon County area. Psychosocial Rehabilitative Services are defined as social, educational, vocational, behavioral, and cognitive interventions provided by members of a clients therapeutic team that address deficits in the individuals ability to develop and maintain social relationships, occupational or educational achievement, independent living skills, and housing, which are a result of a severe and persistent mental illness. Service coordination involves accessing medical, social, educational, and other appropriate services that will help an individual achieve a quality of life and community participation acceptable to the individual. Education: Requires Bachelors Degree in behavioral, social sciences, humanities, or education field. Experience: Prefer experience working with children and families; Requires knowledge of childhood development. Hours of Work: Generally, MondayFriday 8am-5pm; adjustments in work hours warranted to meet the needs of the program. Crisis on Call work required as agency need dictates. Licenses/Certifications: Ability to be credentialed as Qualified Mental Health Professional. Valid drivers license and are insurable by agency insurance. Continued employment is contingent upon successful completion of Adult Needs and Strengths Assessment (ANSA) certification. Physical Requirements: Light lifting under 15lbs., moderate carrying 15-44lbs., simple grasping, walking, sitting, standing, operating a motor vehicle, operating office equipment including a computer, ability to see and hear (with aid), ability to read, write, and count, ability to maintain confidentiality, ability to work closely with others, and the ability to pass the Preventative Management of Aggressive Behavior (PMAB) Training as well as all other physical requirements for this position.
