District 19 Community Service Board LPN - ACT Team in Petersburg, Virginia

District 19 Community Services Board is seeking a full time Licensed Practical Nurse (LPN) for the ACT team. ACT is a highly coordinated set of services offered by a group of medical, behavioral health, peer recovery support providers and rehabilitation professionals in the community who work together as a team to meet the complex needs of individuals living with severe and persistent mental illness.

The incumbent will assess physical health needs, make appropriate referrals to community physicians, providing management and administration of medication in conjunction with the psychiatric provider and ACT lead nurse, providing a range of treatment, lab services, and support services.

Incumbent must have a strong commitment to persons with a severe and persistent mental illness to help them to live in normal community residences, work in market jobs, and have access to helpful, adequate, competent, and continuous support and services.

This position will travel throughout the D19 CSB catchment area: Colonial Heights, Dinwiddie, Emporia, Hopewell, and Petersburg, and the counties of Dinwiddie, Greensville, Prince George, Surry, and Sussex. Travel to satellite clinics required, with agency vehicle provided.

Required:1 year of Psychiatric Nursing experience OR at least 2 yearsof non-psychiatric experience. Must have a current State of Virginia license to practice as a Licensed Practical Nurse. Required to obtain and maintain CPR certification. Must possess a valid VA Drivers License with an acceptable motor vehicle record of -6 or higher Must be able to provide phlebotomy services, administration of intramuscular medication, supportive counseling, have knowledge of psychotropic medication uses, side effects, and potential interactions. Knowledge of community-based services required as well as some out of office work. Must have strong communication and documentation skills. Must have the ability to tolerate light lifting, bending, stooping, reaching, and climbing stairs.

Preferred:Candidate has the skills and competence to establish supportive, trusting relationships with persons with severe and persistent mental illnesses. Respect for client rights and personal preferences in treatment are essential. Work is performed under the general direction of the Medical Services Manager.

Salary:This is a Grade S-C7, starting salary is $50,068 Salary is negotiable
