Description

Under general supervision, the resettlement case manager provides intensive case management to all newly arrived resettlement clients. Working in concert with agency staff, volunteers and external partners, the resettlement case manager ensures that refugee families receive required resettlement services in compliance with national resettlement standards as well as CCFCS case management principles. Effectively supports ongoing and collaborative communication amongst all program staff and is responsible for ensuring a trauma informed care environment.

Essential Duties and Responsibilities

  • In close coordination with resettlement program team members ensures and documents the timely provision of all resettlement services per the standards and requirements outlined by federal refugee resettlement governing bodies.

  • Prepares friends & relatives of refugee families to assist in the delivery of resettlement services.

  • Provides airport reception services.

  • Provides and/or coordinates basic needs requirements – food, clothing, home furnishings and personal items.

  • Facilitates refugee placement in suitable housing.

  • Conducts intake, orientation and assessment of newly arrived refugees.

  • Formulates service plan and coordinates all resettlement service provision according to requirements.

  • Facilitates and coordinates refugee access to identified services providers in the fields of health, education, public benefits, employment, and other community resources as described in service plan.

  • Conducts home visits and home inspections to ensure welfare of clients as well as adequacy of services, housing and basic needs support.

  • Secures payments to vendors and clients and maintains individual client direct assistance budgets.

  • Documents all above mentioned services in client case logs and maintains individual case files in strict accordance with funder and agency requirements.

  • Adheres to agency case management principles and resettlement program service delivery philosophy according to agency policy and internal program manuals.

  • Participates in program, departmental and agency initiatives aimed at quality improvement and routine program maintenance.

  • Attends weekly resettlement outreach service coordination meetings, supervisory meetings and other inter-departmental and agency staff meetings and trainings.

  • Assists program management and other staff in preparation of routine reports and in the completion of various program related administrative tasks.

  • Participates in strategic planning, program improvement work groups, client/stakeholder satisfaction surveys, peer review and other activities as requested by supervisor

  • Participates in relevant Agency meetings and/or trainings.

  • Actively pursues development of professional competencies related to job role performance through reading, supervision, in-service training and attendance at conferences and workshops.

  • Awareness and active support of the Agency's Mission, Vision, Values and Strategic Plan. Including, but not limited to, supporting the Agency's Core Competency initiatives of being Collaborative and Community Focused, Innovative and Entrepreneurial, and Culturally Competent and Diverse

  • Other duties as assigned.

Qualifications

Education: Bachelor’s Degree in Social Work, Human Services, or related field.Equivalent combination of education and experience will be considered.

Credentials: Valid and clean NYS Driver’s Licenseand use of a registered and reliable vehicle.

Experience: At least one year of experience in the Human Services field and/or case management experience. Experience working with volunteers, refugees, and local service providers preferred. Ability to speak English and at least one additional language is also preferred.
