Job purpose

The Behavioral Health Counselor is responsible for providing direct behavioral health services in home, clinic, and field setting to adults and older adults. This position provides services that include providing individual and group therapy and performing case management services. The Behavioral Health Counselor delivers services with attention to cultural relevance and sensitivity.

Duties and responsibilities

Provide culturally sensitive, client-centered individual, group, and family counseling and rehabilitation services to adults and older adults.

? Facilitate referrals to community resources and performs case management services.

? Develop culturally relevant assessment and treatment plans.

? Complete all documentation per Medi-Cal, Santa Clara County and AACI standards within expected deadlines.

? Participate in clinical supervisions, consultations, team meetings, trainings and on-call after hours rotation.

? Licensed Clinicians only

Pay Details: $71,000.00 to $83,000.00 per year
