Job Summary:

Social workers provide psychosocial care in the hospital setting. Social workers link patients with community and agency resources. Social worker engage patients with the necessary social and emotional means for a safe discharge.

Essential Functions:

  1. Primarily responsible for providing consultation services for mental health and substance use concerns, including assessment, crisis intervention, brief counseling and education for patients/families

    1. Assist providers in determining appropriate patient dispositions and facilitate referrals to treatment and community resources post-discharge
    2. Assist in determining the need for involuntary commitments, and facilitate involuntary commitments in collaboration with attending physicians
    3. Provide assistance with assuring safety for behavioral health patients (i.e. managing development and delivery of policies, managing staffing needs in coordination with charge nurses and directors, etc.)
    4. Participate in hospital and community initiatives which address mental health and substance abuse issues
    5. Provide expertise and knowledge in mental health and substance abuse issues to colleagues during interdisciplinary rounds/team meetings as well as through in-service trainings and support if needed
    6. Fill in as a case manager/discharge planner, providing care coordination and discharge planning for primary medical patients on med/surg floors and in ICU (i.e. arranging aftercare services such as home health and SNF)

Bachelor's degree is required

Master's degree is preferred
