Clinic Care Coordinator-RN Location: Fort Defiance, AZ Duration: 13 Weeks Shift Information: M-F, 8-5/730-430, no holidays no weekends. Maximum of 2 Days Call off (Unapproved RTO) during contract. Description: Primary Care Clinic at Client Requirement: Three (3) year Case Management Experience. Collaborate with support nursing staff and with providers for patient care.
