Full-Time position for a Licensed Social Worker (LSW) 11am-7:30pm

The Social Services Designee/ Discharge Planner is responsible for assisting and coordinating with the facility's Social Service Department to streamline the process for resident/guest discharge planning.

Responsibilities may include:

  • Meeting the resident/guest on or shortly after the admission and following them until they discharge to the community.

  • Attend the 72 hour care plan meeting

  • Ordering equipment for the community setting

  • Arranging home health nursing for after discharge, as needed

  • Coordinating the medications for community use

  • Coordinate the entire discharge, with IDT involvement

  • Attending care conferences for sub-acute residents/guests

  • Attending Medicare Meetings

  • Assist in procuring any item related to the discharge process for residents/guests anticipated to be returning to the community.

  • Initiate and coordinate completion of the Post Discharge Plan of Care

  • Develop an individualized discharge care plan for the resident/guest discharge in collaboration with the facility, the family, and the practitioner, assuring the resident/guest receives the care that will meet their needs in the community.

QUALIFICATIONS

  • Minimum of Bachelor’s Degree in Social Work.

  • Licensed Social Worker (LSW)
