SCL Health Clinical Pharmacist in Wheat Ridge, Colorado

Job Description:The Clinical Pharmacist demonstrates consistent application of knowledge and skills to deliver appropriate, safe, efficient, effective, and cost effective pharmaceutical care in the scope of their particular assignment. This position is responsible for the care of patients as well as the development of effective, professional relationships within the multidisciplinary team.

Schedule This clinical pharmacist position is for a variable shift pharmacist who will mainly work on general medicine and general surgery floors within our hospital. This is for a 0.75 FTE, working 30 hours a week. This position is hourly. This shift will work mainly day shifts, with an average of 3 evening shifts per 6-week schedule to assist our team in covering set evening shift vacations/time off. This position will rotate every 3rdweekend staffing. This position will be in the overnight coverage rotation, with a need to cover overnights occurring on average about once stretch a year. This position will be in our holiday rotation, covering 1 major and 1 minor holiday each year

Scope

As a Clinical Inpatient Pharmacist, you need to know how to: Operates from a strategic perspective and assumes overall accountability and authority for respective areas that enhances the overall performance of SCL Health Pharmacy Services and the organization. Facilitates operational improvements, workflow optimization and organizational change through collaborative work relationships to enhance performance across the pharmacy team. Facilitates collaboration to share and implement best practices. Responsible for the implementation of SCL Health Pharmacy Services and SCL Health Medical Group programs and services. Develops clinical pharmacy programs according to relevant policies and regulations. Reviews records of patients to determine the appropriateness of medication therapy, and to identify patients for program outreach. Evaluates patients conditions and any barriers to medication compliance to ensure all issues are being treated. Identifies untreated health problems and refer patients to appropriate physicians or other clinical staff. Develops effective medication plans that minimize the risk of adverse side-effects. Provides medical staff with appropriate medication information for new and existing drug therapies to support safe prescribing practices. Assess the results of pharmaceutical treatments. Collaborates with healthcare professionals to ensure optimal patient care. Advises on the correct procurement, storage, and administration of drugs in the clinic setting. Prescribe and discontinue medications, adjust medication doses, and order labs in accordance with any governing collaborative practice agreements with authorization from the patients physicia Keep accurate documentation of medication plans and patient progress to meet requirements for efficient patient care and to support billing practices. .

Minimum Qualifications

  • Graduate of an accredited College of Pharmacy with a PharmD. Degree or a five (5) year, required. Licensure by the state of Colorado to practice pharmacy, required. Qualified to practice Drug Therapy Management under the Rules Regulations of the practice state (Colorado), required. Current BLS certification endorsed by the American Heart Association, required. Inpatient Pharmacy experience or a PGY-1 Residency, preferred. PGY2 residency welcomed. Clinical experience in a busy hospital setting is necessary for this role.

The hourly range for this position is listed below. Actual hourly rate dependent upon experience.$55.00 - $91.64
