Description

The Nurse Care Manager shall contribute to implementation, coordination, and oversight of each OHH member’s Plan of Care/ITP, assist in the coordination of care with outside providers, and communicate barriers to adherence as appropriate to the team, including the Clinical Director. The Nurse Care Manager shall be involved in overseeing and/or participating in all aspects of OHH services.

This can be a full time position, working 40 hours per week or a part time position, working 20 hours per week.

Because the Nurse Care Manager will occasionally drive to a client’s home in order to provide services, proof of vehicle insurance at $50K/$100K and a valid Maine driver’s license are required.

Duties include and are not limited to:

  • Participating regularly in treatment planning meetings and in organization staff meetings

  • Oversight and responsibility for specific tasks such as initial and random urine drug screening of OHH clients, urine pregnancy screening of OHH clients, coordination with laboratory services, communication with pharmacies, PCP offices and other members of the client’s whole health care team.

  • Review and signature of a medication contract for each OHH client.

  • Care coordination for OHH clients to ensure that their whole health needs are being addressed.

  • Providing regular education to staff and clients according to medical and substance use needs.

  • Attending all OHH team meetings.

  • Obtaining all necessary medical documents and signed releases for all medical diagnosis, lab orders and results, history and physicals, hospitalizations, hospital discharges when applicable.

  • Accepting shared responsibility and authority with other team members to successfully accomplish the goal of each client’s plan of care and other teams.

  • Working harmoniously with all agency staff and community members.

  • Performing required amount of documentation in a timely fashion.

  • Assuring quality in work performed in order to facilitate the delivery of quality services

  • Receive notifications from HealthInfoNet (HIN)

  • Follows up with client, case manager, and PCP after receiving HIN notification and completing necessary documentation

  • Other duties as assigned

Qualifications

Position Requirements:

  • Proof of vehicle insurance at $50K/$100K and a valid Maine driver’s license are required

  • Excellent verbal, written, interpersonal, communication skills

  • Must be 18 years of age or older

  • Must be a registered nurse who has completed the Substance Abuse and Mental Health Services Administration (SAMHSA) required training for an X-DEA license; or a psychiatric nurse licensed as a registered professional nurse and certified by the American Nurses Credentialing Center (ANCC) as a psychiatric and mental health nurse (PMHN); or a Licensed Practical Nurse (LPN); or an advanced practice nurse, as defined by the Maine State Board of Nursing.
